Nomination
Do you know someone worthy of recognition?
There are pharmacy heroes all around us. But often they can be humble, acting on the behalf of others without favour, and so their stories remain untold.
The Scottish Pharmacist Awards is Scotland’s most prestigious, playing a fundamental role as a vehicle to celebrate and acknowledge the remarkable contribution and achievements of pharmacy individuals and teams.
They provide you with an opportunity to recognise deserving colleagues and/or peers.
If someone has inspired you through their dedication, innovation or leadership, show them how important their work is by nominating them for a Scottish Pharmacist Award.

The pharmacist must demonstrate first and foremost that they are patient facing in their role, and are included in both the decisions and governance of practice prescribing to improve patient care. Management of Long Term Conditions
The Chronic Medication Service is one of the core elements of the community pharmacy contract in Scotland. Pharmacists are now tasked with care planning and sometimes serial dispensing for patients, putting them at the front of patient care. For the first time patients with long-term chronic conditions are now primarily managed by pharmacists on a national scale.
We want to recognise a pharmacist or pharmacy team which has embraced the Chronic Medication Service, and which will have delivered continuously high-quality care, with evidence of improved outcomes for individuals. Are you or do you know someone who is excelling in delivering CMS?
